How and where do you order a plain, no frills 10 USD (ish) DSL circuit? I
understand it to be possible, but don't know how to do it...

Try one of the CLEC providers, such as Speakeasy. Unless things have

changed, Speakeasy will accommodate that all across their service
area. They will also definitely do it for residential customers.

I think Covad might do it as well, but now that they're focusing
mostly on businesses rather than reselling to residential ISPs, I'm
not sure that they're signing up residential customers anymore.
