I had my house networked and the person install telephone jacks instead
of wallmounts. I am now planning to remove the jacks via blade and
install wallmounts the only problem is I do not know how.

Do I need to install a box, then the faceplate and terminate or screw
the faceplate with the terminators into a plate or the plater board
directly?

Could someone please point me in the correct direction, are there any
articles on how this is done?
